Position 1
Geriatric Case Manager
Baker Senior Center Naples, a nationally accredited senior center, is seeking a Geriatric Case Manager to develop and facilitate delivery of clinical Case Management services that support clients in an individualized, person-centered manner.
Responsibilities
Work with Older Adults and their families through long-term or short-term case management with the goal of supporting successful independence in the community.
Develop a comprehensive assessment of clients
Development an active and individualized care plan with clients that identify their goals and objectives
Home visits and ongoing contact with clients on your caseload
Provide Information and Referral to community individuals seeking resources
Make linkages and support clients with those connections as needed
Ongoing evaluation of the impacts and outcomes of case management interventions
Provide Case Management support to individuals and families enrolled in other programs at Baker Senior Center Naples
Work within a team setting to consult and support work and share resources to facilitate the best outcome for client
Essential Qualifications
A degree in Social Work, MSW Preferred
Experience in case management or related work
Experience working with Older Adults
Familiarity with Medicare and Medicaid
Knowledge of community resources
Must have excellent time management skills
Able to develop a clear and goal-oriented plan of care
Ability to be creative and innovative
Must possess excellent customer service skills
