Position 1
Geriatric Case Manager
Baker Senior Center Naples, a nationally accredited senior center, is seeking a Geriatric Case Manager to develop and facilitate delivery of clinical Case Management services that support clients in an individualized, person-centered manner.

Responsibilities

  • Work with Older Adults and their families through long-term or short-term case management with the goal of supporting successful independence in the community.

  • Develop a comprehensive assessment of clients

  • Development an active and individualized care plan with clients that identify their goals and objectives

  • Home visits and ongoing contact with clients on your caseload

  • Provide Information and Referral to community individuals seeking resources

  • Make linkages and support clients with those connections as needed

  • Ongoing evaluation of the impacts and outcomes of case management interventions

  • Provide Case Management support to individuals and families enrolled in other programs at Baker Senior Center Naples

  • Work within a team setting to consult and support work and share resources to facilitate the best outcome for client

Essential Qualifications

  •  A degree in Social Work, MSW Preferred

  • Experience in case management or related work

  • Experience working with Older Adults

  • Familiarity with Medicare and Medicaid

  • Knowledge of community resources

  • Must have excellent time management skills

  • Able to develop a clear and goal-oriented plan of care

  • Ability to be creative and innovative

  • Must possess excellent customer service skills
